Section 504 Services

Section 504 of the Rehabilitation Act of 1973 is a civil rights law which prohibits discrimination against individuals with disabilities. Title II of the Americans with Disabilities Act of 1990 (ADA) extends this prohibition against discrimination to the full range of state and local government services, programs, or activities. The Section 504 Process: From Request to Meeting

Every campus has a designated Campus Section 504 Coordinator who serves as your main point of contact throughout this process.

Step 1: Contact Your Campus 504 Coordinator Reach out to your student's campus 504 coordinator to formally request an evaluation. Submit any outside medical or clinical documentation that diagnoses or details your student’s disability or health condition.

Step 2: Complete the Consent & Parent Input Forms The campus 504 coordinator will send you an official Consent for Evaluation form along with a Parent Input Form. Returning these completed forms promptly allows the evaluation window to officially begin.

Step 3: Data Collection Once signed consent is received, the coordinator gathers information from a variety of sources to evaluate your child’s needs. This can include teacher input, classroom work, attendance, standardized test scores, and any medical information provided.

Step 4: Meeting Scheduled The campus 504 coordinator will schedule an initial Section 504 evaluation meeting at a mutually agreed-upon time.

Step 5: Section 504 Meeting & Committee Review A knowledgeable team gathers to review all collected data and determine eligibility. This committee includes:

  • You (the parent/guardian)
  • Campus 504 Coordinator
  • General Education Teacher representative
  • Administrator 
  • Student (if age appropriate)
  • Other relevant campus professionals depending on individual student needs (e.g., School Nurse, Counselor)


If eligible, the committee will collaborate during the meeting to design and finalize an individualized Section 504 Accommodation Plan tailored to support your child.

Child Find

Section 504 of the Rehabilitation Act places the responsibility for identifying and locating students with disabilities on the school. A school/district must annually undertake efforts to identify and locate every qualified individual with a disability residing in the school's jurisdiction who is not receiving a public education. 

If you suspect a child has a disability that requires services under Section 504 of the Rehabilitation Act, please contact your child's home campus. Parents of children attending private schools or home schools can also request a Section 504 evaluation by contacting their child's home campus, as well.
