Part 4: AISD Programs & Resources - What Happens When Checklist

First Meeting - Initial IMPACT Referral

  • Teacher shares IMPACT Student Referral Form, including strengths, concerns, and data.
  • IMPACT team members ask questions regarding the referral to obtain more specific information.
  • IMPACT team sets measurable and reasonable goals for the student.
  • Team consults resources (see attached) and identifies strategies by which to attain goals.
  • Team develops the Austin ISD IMPACT Action Plan, including the following:
    • Intervention goal
    • Intervention strategies
    • Person(s) responsible for intervention
    • Frequency and duration of intervention
    • Instructional grouping
    • Progress review date
  • Teacher and/or interventionist implement action plan for specified time period.
  • IMPACT team reconvenes for the progress review report.

Second and Subsequent Meetings - IMPACT Progress Review Meeting

  • Teacher and/or interventionist review Austin ISD IMPACT Action Plan, share student progress toward goals and record progress on IMPACT Progress Review Report. Progress Monitoring Instrument (reading, math, speech, and/or behavior) must be included.
  • IMPACT team decides to do one of the following:
    • Discontinue the action plan due to the student's adequate progress, but continue to monitor the progress of the student
    • Continue the action plan and monitor student progress for an additional, agreed-upon period of time
    • Modify the action plan and complete a new Austin ISD IMPACT Action Plan
    • Consult with a specialist (i.e., AISD curriculum specialist, behavioral specialist, speech and language pathologist) and modify the action plan based on the specialist's recommendations
    • Consider 504 services
    • Refer the student for special education services Note: Ensure that vision and hearing screening has been completed within the last year.
  • Record the decision of the IMPACT team onto the IMPACT Progress Review Report
