By submitting this application I understand that:

1. AISD will pay the District’s required rate to my selected provider.
2. Services for my child will end when this amount has been reached or no later the June 30, 2009.
3. I will be notified when this designated amount has been reached, and should I wish to continue these services, I will be responsible for any amount exceeding the Districts required rate.
4. Transportation to and from this Supplemental Education Service provider will be my responsibility.
5. If I cancel the service with this provider during the current school year, my child may still be able to receive tutorial services on this campus, or from another state-approved provider.
